The Clinical Transformation Manager- National Accounts, is responsible for managing the ongoing clinical relationships with National Purpose-Built Clinics (PBCs) and Aggregator systems that participate in UnitedHealthcare’s (UHC) Value Based Care Programs.
The selected candidate will oversee and drive Stars, Patient Experience and Accurate Documentation Diagnosis and Coding activities with internal and external leaders supporting the PBC / Aggregator National Programs. In addition, the Clinical Transformation Manager will collaborate on data exchange efforts, provide education, and train external partners on available tools, resources and reporting to help drive and support improvement activities at an ACO and practice level.
As the single point of clinical contact for the National PBC / Aggregators, the Clinical Transformation Manager will be responsible for communicating progress towards achievement of targeted goals to the ACO National Medicare Strategy and Operations Leadership within UnitedHealthcare.

Primary Responsibilities:
- Accountable for successful deployment and management of UHC’s Medicare and Retirement Value Based Care program at the National level
- Develop and effectively maintain solid relationships with the National leadership for the PBC / Aggregators, along with key clinical influencers actively involved in practice transformation
- Support clinical data and interoperability exchanges via available health information exchanges and market-wide platforms
- Analyze national reporting and present process improvement strategies to help identify market level opportunities critical to the success of the Value Based Care Program
- Design practice transformation action plans and implement performance improvement initiatives designed to assist the ACO in achieving contractually required transformation milestones
- Regularly facilitate clinical discussions with National PBC / Aggregator leadership, presenting compelling and effective analyzed trends, progress on the transformation action plan and contractual goals
- Use data to analyze trends and work with stakeholders to agree on and implement proactive strategies to address issues, and measure impact using a PDSA rapid cycle improvement approach, including external practice data
- Assist in creating work flows to optimize care delivery, introduce best practices improvements, and evaluate outcomes using rapid cycle improvements PDSA methods to reach mutual goals
- Educate and deploy technology tools to support Practice Transformation
- Build and effectively maintain relationships with team members in the UHC Value Based Care organization as well Medical Directors and local Network leads in support of the program
- Accountable at the National level to communicate ACOs overall contractual performance updates to both to internal
- Consult and partner with internal UHC matrix partners and the practice to identify organizational and structural challenges hindering achievement of desired program outcomes

Years of post-high school education can be substituted/is equivalent to years of experience.
Required Qualifications:
- 3+ combined years of experience with improving clinical quality, establishing best practices, health care analytics, or driving clinical transformation initiatives with population health programs, patient center medical homes or in the managed care industry
- 2+ years of experience in interpreting and utilizing clinical data analytics, outcomes measurement in healthcare and use of that data to drive change
- 1+ years of experience working as a Clinical Transformation Manager within local markets
- 1+ years of collaborating with providers on Medicare HEDIS and STAR measures, CAHPS/HOS, or other value-based quality measures
- Experience implementing clinical practice transformation initiatives designed to help provider or hospital groups achieve large-scale health transformation goals
- Experience working with and collaborating successfully with senior level leadership
- Experience leading groups and solid presentation skills
- Proven excellent relationship building skills with internal and practice teams to drive goal alignment
- Proficient in using Microsoft Office – Word, Excel, Outlook, and PowerPoint
- Access to high-speed internet for home office set up
Preferred Qualifications:
- Physician practice experience
- Experience preparing and presenting information to clinical and executive level leadership

California, Colorado, Connecticut, Nevada, New Jersey, New York, Rhode Island, or Washington Residents Only: The salary range for California, Colorado, Connecticut, Nevada, New Jersey, New York, Rhode Island or Washington residents is $85,000 to $167,300 per year. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
