Position Description
Senscio Systems is a Harvard, MA based company with current operations in MA, NH, ME, PA, RI, CT, NC and FL. The company’s healthcare division, Ibis Health, is a technology-enabled, AI Assisted Care Management Platform that our Nurse Practitioner-led teams of patient-facing care advocates leverage to provide care plan-based care coordination, remote patient monitoring, and home-based self-management coaching to patients in collaboration with their primary care providers.
Ibis Health’s 24/7 home-based virtual care coordination platform serves as an extension of each patient’s PCP practice, acting on guideline-based RPM threshold triggers and actionable alerts related to vital signs monitoring and medication adherence. The team monitors and integrates physical and behavioral health into patient-centered plans of self-management, and the easy to use, AI based technology, facilitates timely intervention, efficient documentation, administration, and meaningful stakeholder communication.
Use of the Ibis Health platform has documented significant reductions in avoidable hospitalizations and overwhelmingly positive patient experience scores, and we continue to innovate strategies to develop care management best practices for patients with chronic conditions.
We are currently recruiting a part-time physician to provide 15 minute telehealth patient intake assessments. This is an outstanding opportunity for physicians with multi-state licensure and those seeking additional hours. Includes a generous compensation package.
