Our passion is leveraging a unique, patented AI technology paired with a collaborative multi-disciplinary clinical team to support patients (we call them Members) with complex medical challenges. As a provider member of our team, you will leverage the virtual medicine platform to support patient self-management, while ensuring PCPs are kept aware of changes in their patient’s health status.
Senscio’s 24/7 home-based virtual medicine program acts as an extension of our member’s primary care team, providing their PCP and associated specialists w/ actionable situational awareness. Integrating physical and behavioral health needs with a broad scope of social determinants of health needs, we provide a truly holistic, patient-centered plan of self-management support. The AI based technology allows for easy and efficient administration and you will spend much less time documenting and more time directing the care of your patient.

- Location: Preferred in New England however some flexibility. Active license in Massachusetts a requirement with licensure in additional States (FL, RI, ME, NH, PA, CT, NC) a strong plus.

Minimum Qualifications
- Board Certified Nurse Practitioner with experience in Primary Care, Family Practice and/or Geriatrics.
- Minimum of Master’s Degree in Nursing.
- APRN license in good standing in Massachusetts w/additional licensure in Florida, Maine, New Hampshire, Rhode Island, Pennsylvania, North Carolina and Connecticut a plus.
- 10+ years of experience managing complex patients.
- Willingness to obtain licensure in all states/regions where Senscio has operations.
- Experience/interest in telehealth service delivery.
- Passion for providing Chronic Care Management.
