Position Description

Senscio Systems is a Harvard, MA based company with current operations in MA, NH, ME, PA, RI, CT, NC and FL. The company’s healthcare division, Ibis Health, is a technology-enabled, ai Assisted Care Management Platform that our Nurse Practitioner-led teams of patient-facing care advocates leverage to provide care plan-based care coordination, remote patient monitoring, and home-based self-management coaching to patients in collaboration with their primary care providers.

Ibis Health’s 24/7 home-based virtual care coordination platform serves as an extension of each patient’s PCP practice, acting on guideline-based RPM threshold triggers and actionable alerts related to vital signs monitoring and medication adherence. The team monitors and integrates physical and behavioral health into patient-centered plans of self-management, and the easy to use, AI based technology, facilitates timely intervention, efficient documentation, administration, and meaningful stakeholder communication.

Use of the Ibis Health platform has documented significant reductions in avoidable hospitalizations and overwhelmingly positive patient experience scores, and we continue to innovate strategies to develop care management best practices for patients with chronic conditions.

We are currently recruiting a Physician Partner to provide expertise, guidance, and supervision to the multi-disciplinary Ibis Health team that functions in concert and communication with the Pennsylvania PCPs whose patients we follow.

Specific responsibilities for this role include establishing, reviewing and periodic updating of care plans, providing general oversight and supervision of the care provided by the Ibis Health team of APRNs, RNs, LCSWs, CCM Specialists, and Member Advocate professionals.

Minimum Qualifications

  • Demonstrated clinical experience in Primary Care; FM, IM and/or Geriatrics preferred.
  • Experience in health team guidance, oversight, and supervision in patient care delivery.
  • Active license in good standing in Massachusetts/PA and current credentialing with Medicare and Medicaid. Credentialing with commercial payers is a plus.
  • Willingness to participate in medical licensure compacts for credentialing in additional States to support the company’s expanding growth.
  • Experience/interest in the use of digital health platforms in care coordination and interprofessional communication.
