Our passion is leveraging a unique, patented AI technology with a collaborative multi-disciplinary clinical team to support patients (we call them Members) with complex medical challenges. As a key member of the Senscio team, you will be in the central role of providing real-time self-management support to our Members to enable them to continue to live comfortably and safely in their homes and communities.

Senscio’s 24/7 home-based virtual program acts as an extension of our Member’s primary care team, providing their PCP and associated specialists w/actionable situational awareness. Integrating physical and behavioral health support with a broad scope of social determinants of health needs, we provide a truly holistic, patient- centered plan of self-management support. We have shown dramatic improvement in clinical outcomes and overwhelmingly positive Member experience scores, leading to tremendous projected growth in membership.

Position Description

Our Member Advocate is at the center of the program; closely connected across our multi-dimensional care team and providing direct support to our Members. The AI based technology allows for easy and efficient administration and you will spend significant, quality time connected with your panel of Members, helping them live their daily lives with the highest quality of life possible.

While currently a fully remote position, It is anticipated that it will transition to a hybrid structure with some time in an office, particularly during the first 90 days of employment, and then move to a schedule of some in office and some days working from a remote location of choice. With this in mind location within a commuting distance of Harvard, MA or southern Maine is a necessity.

As a Member Advocate, your day will begin with a review of tasks, issues, or requests from or about your panel of Members. These items are prioritized and provided through our unique technology platform, with which your Members are engaged. You will be observing their success and progress in completing the tasks assigned to them through their personalized care plan, and coaching as necessary and needed. If and when you observe an event that is outside the realm of the routine “preventative” tasks and coaching, such as an exacerbation of an underlying condition or signs of an emerging health challenge, you will escalate the information to the next level of our care team for triage and care. If social challenges such as food insecurity, transportation, getting new supplies of prescriptions are being experienced, you will coordinate with the local community based organization to assist your Member in resolving the issue. By developing strong, trusting and meaningful relationships through value added support to your Member, you will truly be changing lives.

Qualifications

  • Medical Assistant/CNA/Community Health Worker certificate and experience required.
  • Energetic, self-starter with a demonstrated work ethic.
  • Computer/technology savvy.
  • High level of energy and capable of/willing to work in a fast moving, rapidly changing environment.
  • Strong communication skills/willingness to build trusted relationships quickly.

Ability to work independently and remotely, but as part of a highly integrated and collaborative team. Past remote work record of success highly valued.
