This Practice Liaison II will be responsible for:

Establishing strong relationships between the Hospital and referring physicians and their practices. Interfacing with both physicians and their staffs to improve communication and understanding of the needs and wants of the referring practices. Informing referring practices regarding hospital policies or updates. Keeping in contact with the referring practice to resolve any outstanding issues. Serving as the primary contact for the referring providers of the PPOC, National Referring Line, BCHP, Grand Rounds Online and BCH Online Second Opinion Platforms and Pinnacle Care Concierge Services. Communicating missed appointments to referring providers to ensure medically compromised patient receive care according to medical home protocols and reschedule as needed for the families. Keeping in contact with the referring practice to resolve any outstanding issues. Monitoring the online request an appointment database and respond to the requests within 24 hours. Scheduling directly or put the family in touch with the appropriate department. Monitoring the scheduling DL for incoming PCP update requests. Updating the PCP as needed so that providers can access their patient records in the My Children’s portal. Monitoring and supporting the Grand Rounds (GR) platform. Answering queries from the GR coordinators, BCH providers and department administrators. Uploading medical records from the GR portal. Providing Life Image links for the GR coordinators and push imaging to the BCH image system. Creating medical records for all GR online opinions (domestic or international) utilizing GR demographic spreadsheets. Serving as the point person in the absence of supervisor for problem identification and resolution, as well as notifying manager when confronted with difficult and/or unpredictable situations with patients, referring practices and team members.

To qualify you must:

High School Diploma required. Minimum of 18 plus months experience as a Practice Liaison I OR 3 years of related experience i.e. Customer service experience, preferably in a call center setting. Scheduling experience in a healthcare setting would be a plus but not required. Basic computer navigation and keyboarding skills, including ability to multi-task and navigate between multiple systems. Strong and accurate data entry. Remain current on all scheduling changes and updates for direct scheduling departments. Ability and willingness to work a flexible schedule as needed. Ability to develop and maintain effective interpersonal relationships. Strong knowledge of Microsoft Office Suite. Advanced experience with Epic Scheduling and Power Chart Medical Records. Intermediate skill in collecting and analyzing data.
